Community Mental Health Center, Inc. (CMHC) provides Wraparound services to children and their families in Southeastern Indiana. CMHC is the Access Point for Wraparound services under both the Children's Mental Health Initiative through Department of Child Services and the Children's Mental Health Wraparound through Division of Mental Health and Addiction. If eligible, families can choose Wraparound services through CMHC or Choices Coordinated Care Solutions.  Please use this form to refer for services and the family will be contacted in order to be screened for eligibility.  /uploads/REFERRAL FOR IFS SERVICES CMHC.docx
